About the Role

This exciting opportunity has arisen for a skilled and compassionate Adult Community Inclusion Support Worker to join our multi-disciplinary team at Surrey and Borders Partnership NHS Foundation Trust.

As a key member of our Forensic Outreach and Liaison Service, you will play a vital role in creating a safe, supportive, and accessible environment for our clients.

You will provide time-limited 1:1 support, offering information and guidance to enable clients to access a wide range of agencies and services that can provide help and advice.

Main Responsibilities
  1. Develop and implement an assertive outreach approach to engage with clients in the community.
  2. Support clients in community activities and education, promoting their physical, psychological, and social well-being.
  3. Provide increased support to clients transitioning back into the community, ensuring a smooth and successful reintegration.
Key Skills and Qualifications
  • NVQ level 3 in Health Care or equivalent experience.
  • Counselling and/or psychological training and skills would be an advantage.
Experience
  • Minimum of three years experience working within a mental health setting or with people who experience mental illness.
  • Ability to demonstrate experience of working towards an environment that promotes user involvement and recovery.
  • Experience of working in Multi-Disciplinary Teams and/or with other agencies.
  • Ability to identify changes to clinical risk and implement actions to ensure a person's safety.
  • Experience in delivering plans of care that support physical, psychological, and social needs.
What We Offer

We offer a competitive salary of £27,857 to £30,570 per annum, including 5% Fringe HCAS, pro rata.

This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such, a submission for Disclosure to the Disclosure and Barring Service will be required.
