We are seeking a highly motivated and effective Community Homecare Nurse to join our newly formed team in the Thame/Princes Risborough area. As a Community Nurse, you will work closely with a locality team to provide high-quality nursing services to patients in their own homes.

Main Responsibilities:
  • Deliver specialized care to patients with complex health needs in their own homes and in residential care settings.
  • Work collaboratively with social care and primary healthcare professionals to meet the needs of patients.
  • Administer medication, provide end-of-life care, and manage deteriorating patients.
  • Utilize advanced assessment skills and clinical nursing skills to provide holistic care.
