Job Overview
Buckinghamshire Healthcare NHS Trust is seeking a highly motivated and effective Community Nurse to join their team. As a Community Nurse, you will work within a locality team to meet the needs of complex patients in the community and avoid admissions to acute settings.
Main Duties of the Job
As a Community Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes and in residential care settings. This will include medication administration, end-of-life care, managing deteriorating patients, and providing wound care.

Person Specification
- Registered Nurse or student nurse awaiting confirmation of NMC registration.
- Demonstrate experience post or pre-registration placements in relevant clinical areas.
- Ability to communicate effectively with patients, relatives, carers, and other professionals.
Skills, Abilities, and Knowledge
- Knowledge of the NMC Code of Professional Conduct.
- Knowledge of the Data Protection Act.
- Able to work under pressure and manage own caseload.
Special Circumstances
- Car driver/owner able to travel to other locations within the trust.
