Main Responsibilities

To provide skilled and professional assessment of health needs and relevant nursing care management for patients with complex medical conditions.

The ideal candidate will have experience in case management, medicines management, and supporting self-care and independence in patients.

This role requires a strong understanding of professional practice and leadership, as well as experience in managing care at the end of life.

Additionally, the candidate should have the ability to work autonomously, meet deadlines, and work under pressure.

  • Highly developed professional knowledge base, supported by educational development and different work environments
  • Specialist knowledge of governance and risk strategies and their application in a community setting
  • Excellent interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers, and the public
  • Ability to plan and manage resources within allocated budget effectively
About the Role

This is a full-time, permanent position with a salary of £46,148 to £52,809 per annum, pro rata.

The successful candidate will be working within the Buckinghamshire Healthcare NHS Trust, providing care to patients with complex medical needs.

The role requires a valid and current driving license or the ability to travel to locations throughout Buckinghamshire.

Requirements
  • Educated to Degree level or equivalent competencies in health-related subjects
  • Level 1 Registered Nurse
  • Recognised teaching or mentorship qualification equivalent to level 3
  • District Nursing SPQ
  • Relevant postgraduate clinical training
