Community Health Nurse

We are seeking a highly motivated and effective Community Homecare Nurse to join our team in the Thame/Princes Risborough area. As a Community Nurse, you will work within a newly formed team to provide a high-quality nursing service within the District Nursing field.
Main Responsibilities:- Provide nursing care to patients in their own homes, focusing on holistic care and addressing complex health needs.
- Work collaboratively with a locality team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, and Healthcare Assistants to meet the needs of patients in the community.
- Develop and implement individualized care plans, utilizing advanced assessment skills and clinical nursing skills to manage patient care.
- Collaborate with social care and other primary health care professionals, such as GPs, Community Therapies, and specialized services, to ensure comprehensive patient care.
- Registered Nurse or student nurse awaiting confirmation of NMC registration.
- Committed to ongoing professional development to enhance practice.
- Experience in relevant clinical areas, including care of the elderly, community, orthopaedics, neurology, respiratory, palliative care, and long-term conditions.
