Role Overview
We are seeking an experienced Community Support Specialist to join our team at Forward Leeds. In this role, you will work with vulnerable adults, providing holistic support to help them overcome complex challenges.

About the Role
This is a rewarding opportunity to make a real difference in people's lives. As a Community Support Specialist, you will be responsible for assessing and engaging with individuals who require additional support due to multiple needs. You will work closely with the multi-disciplinary team to develop personalized care plans and provide outreach support to reduce barriers to engagement.

Key Responsibilities
* Assess and engage with individuals identified as requiring additional support
* Manage a caseload of service users and review ongoing care and treatment plans
* Conduct home visits, support clients to appointments, and meet individuals in the community
* Work closely with the multi-disciplinary team to provide advice and guidance on care planning and service delivery

Requirements
* Full clean UK driving licence and access to your own vehicle
* Enhanced DBS check
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
