About the Role: We are seeking a highly motivated Healthcare Planning Coordinator to join our team. In this role, you will be responsible for managing a caseload of care packages and coordinating with care teams to deliver high-quality care to service users.

  • • Develop and implement effective care plans in collaboration with care teams and service users
  • • Provide excellent communication and relationship-building skills to ensure seamless care delivery
  • • Work closely with healthcare professionals to address any day-to-day changes or concerns

Requirements: To be successful in this role, you will need:

  • Experience:
  • • At least 1 year of experience in care coordination or a related field
  • • Proven track record of delivering high-quality care services

Salary: We offer a competitive salary of £32,000 - £37,000 per annum, plus bonuses and benefits.

Working Arrangements: This is an office-based role, Monday to Friday, 9 am to 5 pm. Occasional travel across the Merseyside region may be required.
