About the Role

We are seeking a highly motivated and experienced Care Coordinator to join our team. As a key member of the community care team, you will be responsible for assessing the needs of service users, developing and implementing effective care plans, and working collaboratively with multidisciplinary teams.

The successful candidate will have a degree in nursing or a related field and be registered as a Mental Health Nurse or equivalent. They will possess excellent communication and interpersonal skills, with the ability to build strong rapport with service users, families, and colleagues. A strong leader with the ability to motivate and inspire others, the ideal candidate will share our commitment to delivering person-centred care and strive to make a positive difference in the lives of those we serve.

Key Responsibilities

  • Assess and develop effective care plans for service users

  • Participate in quality improvement initiatives to enhance services and outcomes

Salary: £45,000-£60,000 per annum depending on experience
