Adult Care Coordinator

We are seeking an experienced Social Work Specialist to join our team in delivering exceptional adult care services.

Key Responsibilities:- Talk to clients to understand their needs and concerns
- Perform assessments to identify areas for support
- Provide expert advice and guidance
- Collaborate with community resources to deliver exceptional outcomes
To be considered for this position, you must hold a Social Work Qualification and maintain registration with SWE. A minimum of 3 years of experience after qualifications is also required.
