Care Coordinator Specialist

Are you an experienced Care Coordinator looking to take your skills to the next level? This role offers a unique opportunity to work in a dynamic care setting, utilizing your planning, coordination, and problem-solving skills to deliver high-quality patient care.
You will be responsible for managing a caseload of care packages, understanding the varied nature and fast pace of the role. As a skilled Care Coordinator, you'll apply your communication and relationship-building skills to effectively manage rotas for your care teams, respond efficiently to any day-to-day changes, and act as the first point of contact for service users, their families, and other healthcare professionals.
This is an office-based role, Monday to Friday, 9 am to 5 pm, with occasional travel across the Merseyside region required. Applicants must hold a Driving Licence and have access to a vehicle.
Competitive Salary and Benefits- Salary: £30,000 - £35,000 per annum (including payment for on-call)
- Bonus Scheme: up to £2,000 per annum
- Paid DBS
- No requirement to provide hands-on care
