Complex Needs Care Coordinator

About the Role
We are seeking a qualified Complex Needs Care Coordinator to join our team at Independent Living Solutions Ltd. The successful candidate will be responsible for coordinating the care of our client, Elijah, who has complex care needs.
About You
To be successful in this role, you will need:
- Experience working with children with complex care needs
- Experience of coordinating care and managing care teams
- Excellent communication and organizational skills
- Ability to work autonomously and make decisions in a fast-paced environment
- Knowledge of care planning and documentation
- Ability to maintain confidentiality and handle sensitive information
Salary and Benefits
The salary for this role is £25-£30 per hour, dependant on experience. Other benefits include:
- Ongoing training and development opportunities
- Regular clinical supervision and review meetings
- On-going support from a dedicated Case Manager and HR department
- Pension (subject to qualifying status)
- Free DBS check
Work Environment
You will be working in a family home setting, supporting Elijah and his care team. The role will require you to work independently and as part of a team, using your initiative and problem-solving skills to ensure that Elijah's care needs are met.
