Public Health Nurse Practitioner

We are seeking a highly skilled and experienced Public Health Nurse Practitioner to join our Healthy Family Team in central Lancashire.
This is an excellent opportunity to work in a specialist role, delivering high-quality care to families in need.
About the RoleThe successful candidate will be responsible for delivering the Healthy Child Programme, including assessment of health needs, development, implementation, and evaluation of packages of care.
This role requires strong leadership and management skills, with the ability to work effectively in a team environment.
- Deliver the Healthy Child Programme, including assessment of health needs and development of packages of care.
- Use the Family Health Needs Assessment tool to plan and evidence care.
- Devise and implement individualised care plans for each child based on a needs assessment.
- Identify and assess health, growth, and development in the 0-5 age range.
- Use evidence-based practice to develop and maintain a high-quality standard for Specialist Public Health Nurse/Health Visiting practice.
The estimated salary for this role is between £28.00 - £35.00 per hour, dependent on experience.
