About the Role

We are seeking a skilled and compassionate professional to join our team as a Community Transfer of Care Hub Case Manager. As a key member of our multidisciplinary team, you will play a vital role in facilitating the discharge of patients from acute hospitals to the right care pathway.

Key Responsibilities
  • Provide person-centred, holistic assessments to identify the most appropriate discharge pathway for patients.
  • Triage referrals received by the Transfer of Care Hub, liaising with patients, families, and ward staff to determine the best course of action.
  • Collaborate with the multidisciplinary team to discuss complex cases and agree on a joint action and discharge plan.
  • Utilize clinical reasoning and assessment skills to facilitate timely hospital discharge to the most appropriate environment.
  • Effectively triage and prioritize a large caseload, adjusting daily work plans in response to system-wide pressures and escalation.
