Job Summary

We are seeking a skilled and compassionate professional to join our team as a Community Transfer of Care Hub Case Manager. As a key member of our multidisciplinary team, you will play a vital role in facilitating the discharge of patients from acute hospitals to the right care pathway.

About the Role

As a Community Transfer of Care Hub Case Manager, you will be responsible for providing person-centred, holistic assessments to identify the most appropriate discharge pathway for patients. You will work closely with patients, families, carers, and ward staff to ensure a smooth transition from hospital to community care.

You will also be responsible for triaging referrals, utilising your analytical and problem-solving skills to inform decision-making, and coordinating with the multidisciplinary team to agree a joint action and discharge plan.

Key Responsibilities
  • Provide person-centred, holistic assessments to identify the most appropriate discharge pathway for patients
  • Triage referrals and utilise analytical and problem-solving skills to inform decision-making
  • Coordinate with the multidisciplinary team to agree a joint action and discharge plan
  • Work closely with patients, families, carers, and ward staff to ensure a smooth transition from hospital to community care
