Sirona care & health is excited to introduce two innovative Transfer of Care Hubs aimed at enhancing patient discharge processes from hospital to home: one located at our Bristol facility and another at Weston General Hospital. These Hubs will unite various stakeholders, including the acute trust, community partners, local authorities, and voluntary sector representatives, to form a cohesive multidisciplinary team.

The primary objective is to collaboratively facilitate a 'Home First' approach by integrating professionals from diverse organizations to simplify complex discharge scenarios.

We are looking for candidates with experience in community settings and a solid understanding of the clinical acuity levels that can be managed in patients' homes. Additionally, familiarity with rehabilitation pathways, discharge planning, and mental capacity considerations, along with exceptional interpersonal and negotiation skills, is essential.

Key Responsibilities

As a vital member of the multidisciplinary team, you will:

  • Conduct comprehensive, person-centered assessments to facilitate appropriate discharge pathways from acute care settings.
  • Manage and prioritize referrals received by the Transfer of Care Hub, collaborating with patients, families, and ward staff to determine the best discharge options.
  • Engage with the Transfer of Care Hub team to discuss complex cases and develop joint action and discharge plans.
  • Utilize multidisciplinary assessment skills and sound clinical judgment to ensure timely hospital discharges to suitable environments.
  • Effectively manage a substantial caseload, adjusting daily plans to respond to system-wide demands and optimizing community resources.
  • Demonstrate high-level problem-solving abilities by gathering information from various sources and applying professional expertise to guide decision-making, including conducting mental capacity assessments and making best interest decisions when necessary.
