About the Role

The Transfer of Care Hub is a pioneering initiative that brings together professionals from various organizations to streamline complex discharges and support patients to return home safely. As a Case Manager, you will be responsible for providing person-centered assessments, triaging referrals, and collaborating with patients, families, and ward staff to identify the most appropriate discharge pathway.

Key Responsibilities
  • Conduct holistic assessments to facilitate timely hospital discharge
  • Triage referrals and liaise with patients, families, and ward staff to identify the most appropriate discharge pathway
  • Collaborate with the Transfer of Care Hub team to discuss complex cases and agree a joint action and discharge plan
  • Utilize multi-disciplinary assessment skills to facilitate timely hospital discharge to the most appropriate environment
  • Effectively triage and prioritize a large caseload and adjust daily work plan in response to system-wide pressures and escalation
