About the RoleThe Transfer of Care Hub is a pioneering initiative that brings together professionals from various organizations to streamline complex discharges and support patients to return home safely. As a Case Manager, you will be responsible for providing person-centered assessments, triaging referrals, and collaborating with patients, families, and ward staff to identify the most appropriate discharge pathway.
Key Responsibilities- Conduct holistic assessments to facilitate timely hospital discharge
- Triage referrals and liaise with patients, families, and ward staff to identify the most appropriate discharge pathway
- Collaborate with the Transfer of Care Hub team to discuss complex cases and agree a joint action and discharge plan
- Utilize multi-disciplinary assessment skills to facilitate timely hospital discharge to the most appropriate environment
- Effectively triage and prioritize a large caseload and adjust daily work plan in response to system-wide pressures and escalation
