We are seeking a highly skilled Health and Wellbeing Specialist to join our team at Gosforth Jesmond Health, a company dedicated to delivering high-quality primary care services. As a key member of our Integrated Personalised Care Team, you will play a critical role in supporting patients to take an active role in their health and wellbeing.
The ideal candidate will have a minimum of 2 years' experience in delivering weight management services, including working with diabetic patients, in a community setting. They should also have a good standard of general education, including GCSE grade C/Level 5 or above in English Language and Mathematics or equivalent.
This is a new role that requires someone who can work collaboratively as part of our friendly personalised care team, liaising with the practice team, patient/carer, and complex care team as appropriate. The successful candidate will be able to motivate and engage patients through multiple sessions, identifying their needs, setting goals, and supporting them to implement their personalised health and care plan.
