Inclusion, part of Midlands Partnership NHS Foundation Trust, is seeking a compassionate and driven individual to join our Integrated Mental Health and Substance Misuse Service at HMP Isle of Wight.

This rewarding role offers the opportunity to provide comprehensive mental health care and holistic support to service users as they navigate their journey within the prison environment and prepare for successful reintegration into the community.

Your Responsibilities

  • Conduct thorough assessments, develop individualized care plans, implement evidence-based interventions, and evaluate progress for a diverse caseload of service users.
  • Collaborate effectively with a multidisciplinary team, including healthcare professionals, correctional officers, and community partners, to ensure comprehensive and coordinated care.
  • Provide specialized support in release planning, facilitating a smooth transition back into the community by connecting individuals with essential resources and support networks.
  • Engage with service users who may present complex needs, including mental health challenges, substance misuse disorders, learning disabilities, and forensic histories.
  • Contribute to a supportive and collaborative team environment, actively participating in supervision sessions and professional development opportunities.
