As a key member of our team, you will play a vital role in supporting patients to achieve their goals and regain independence in a reablement setting.
Key Responsibilities- Deliver high-quality, patient-centered care in a reablement setting, working collaboratively with the multidisciplinary team to achieve patient outcomes.
- Support patients to develop skills and confidence in daily living activities, promoting independence and autonomy.
- Contribute to the development and implementation of individualized care plans, working closely with patients, families, and healthcare professionals.
- Provide emotional support and guidance to patients and their families, promoting a positive and supportive environment.
- Collaborate with the multidisciplinary team to ensure seamless transitions of care, promoting continuity and coordination of services.
- Education to GCSE level Grade 5 in English and Mathematics or equivalent.
- Care Certificate and/or equivalent healthcare experience.
- Some previous experience in a nursing or care assistant role.
- Good organizational skills, basic IT and computer skills, and excellent interpersonal and communication skills.
