About the Role

We are seeking a highly skilled Registered Nurse with experience in ICU/HDU or complex care to join our team as a Senior Community Care Specialist. As a key member of our community care team, you will be responsible for assessing and reviewing individual care packages, providing practical training to care workers, and clinical supervisions and instructions using your clinical expertise and guidance.

You will work closely with our healthcare workers at the point of care, which may include home, school, community, and day centers. Your clinical knowledge and experience will be essential in interacting with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments.

This is a field-based role, and you will manage your own diary and maintain a work-life balance while giving your clients the opportunity to enjoy a quality life that they deserve in their own home.

Key Responsibilities

Assess and review individual care packages to ensure the best possible outcomes for our clients

Provide practical training to care workers to enhance their skills and knowledge

Conduct clinical supervisions and instructions to ensure high-quality care delivery

Work closely with our healthcare workers to ensure seamless care delivery

Interact with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments to ensure comprehensive care

Requirements

Registered Nurse with experience in ICU/HDU or complex care

Self-motivated, enthusiastic, caring, empathetic, and compassionate individual

Ability to work independently and as part of a team

Strong communication and interpersonal skills
