We are seeking a motivated and empathetic Support Team Member to join our team and provide exceptional support services to adults with learning disabilities. As a valued member of our team, you will work collaboratively with colleagues to deliver person-centred care that prioritizes the needs and preferences of each individual.
Key responsibilities include:
- Providing emotional and practical support to enable individuals to maintain their independence and autonomy
- Helping individuals develop new skills and accessing community activities and resources
- Promoting physical and mental well-being through personalized care plans and interventions
- Collaborating with colleagues to ensure seamless care transitions and effective communication
