About the Role

We are seeking a dedicated and compassionate Healthcare Support Worker to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to our patients.

Key Responsibilities

  • Work alongside our nursing team to support patients through their reablement pathway
  • Conduct observations, physical health checks, and documentation
  • Support patients with their Activities of Daily Living (ADLs)
  • Facilitate escorts off ward and complete IROC using the recovery model
  • Participate in recreational and physical activities to promote independence and daily living skills

Person Specification

  • Previous relevant experience working with people with mental health problems
  • Essential qualifications include the Cavendish Care Certificate and NVQ Level 2 in Care
  • Desirable qualifications include a Certificate in Community Mental Health and ECDL
