Job Title: Nurse Case Manager

Job Summary:

Calibre Global Consulting is seeking a dedicated and experienced Nurse Case Manager to join our team. As a key member of our healthcare services team, you will play a vital role in delivering exceptional patient care and outcomes.

Key Responsibilities:

  • Develop and Implement Care Plans: Collaborate with patients, families, and healthcare providers to create individualized care plans that meet the unique needs of each patient.
  • Coordinate Healthcare Services: Work closely with healthcare providers, social workers, and other professionals to ensure effective and efficient care delivery.
  • Patient Education and Support: Educate and support patients and their families about treatment options, care plans, and health-related information.
  • Collaboration and Communication: Collaborate with healthcare providers, social workers, and other professionals to optimize patient outcomes and ensure seamless care delivery.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date patient records and documentation.

Requirements:

  • Registered Nurse (RN) License: Possess a valid RN license and a strong foundation in nursing principles and practices.
  • Experience in Case Management: Previous experience in case management or a similar clinical role, with a proven track record of delivering high-quality patient care.
  • Strong Communication and Organizational Skills: Possess excellent communication, organizational, and problem-solving skills, with the ability to work independently and as part of a multidisciplinary team.
  • Knowledge of Healthcare Regulations: Stay up-to-date with the latest healthcare regulations, best practices, and quality assurance processes.
