Job Description

**Job Summary**

Leaders In Care Recruitment Ltd is seeking a skilled Nurse Case Manager to join our team in the Merseyside region. As a Community Nurse Case Manager, you will play a vital role in supporting clients with complex needs to live independently at home and within their wider communities.

**Key Responsibilities**

  • Conduct initial assessments in the community to identify client needs and develop personalized care plans.
  • Train and assess workers to ensure they have the necessary skills to provide high-quality care.
  • Manage a caseload of clients with complex needs, providing ongoing support and coordination of care services.
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care delivery.
  • Monitor client progress and adjust care plans as needed to ensure optimal outcomes.

**Requirements**

  • Registered Nurse with a valid nursing license.
  • Experience working with clients with complex needs in a community setting.
  • Strong assessment and planning skills, with the ability to develop and implement effective care plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality care.
