We are looking for an experienced Learning Disability and Mental Health Practitioner to join our team in Shropshire and the West Midlands. You will be responsible for delivering high-quality support services that enable individuals to live fulfilling lives.

Responsibilities

Delivering person-centred support services that meet the needs of individuals with learning disabilities and mental health needs.

">

Working collaboratively with colleagues to develop and implement individualised support plans.

">

Providing training and support to colleagues to ensure they have the skills and knowledge required to deliver high-quality support services.

">

A competitive salary of £35,500 - £40,000 per annum (dependent on qualifications and experience).
