We Manage Jobs (WMJobs) is seeking a skilled Community Mental Health Practitioner to join our team in Telford and Wrekin. In this role, you will work within our local clinical rehab team, delivering rehabilitation services to individuals with mental health needs.

As a Community Mental Health Practitioner, you will be responsible for assessing service users' needs, developing personalized care plans, and coordinating their care packages. You will also work closely with other professionals to ensure seamless transitions between services and effective outcomes for service users.

The successful candidate will have a strong background in social work or a related field, with experience of working with vulnerable adults. You will be passionate about promoting positive mental health and wellbeing, and committed to delivering person-centered care that meets the unique needs of each individual.

Salary: £35,000 - £45,000 per annum, dependent on experience.
