Job Summary:

Key Responsibilities:

  • Collaborate with multidisciplinary teams to coordinate care, ensuring seamless transitions and optimal outcomes.
  • Provide expert guidance on equipment and adaptation solutions, empowering individuals to live safely and confidently in their own homes.
  • Stay up-to-date with the latest research, best practices, and legislative changes, applying this knowledge to improve patient outcomes and inform service development.
  • Develop and maintain effective relationships with service users, carers, and other stakeholders, promoting open communication and trust.
