About the Role

We are seeking a highly skilled and experienced Senior Rehabilitation Support Worker to join our team in Intermediate Care Lambeth. As a Senior Rehabilitation Support Worker, you will play a vital role in providing home-based rehabilitation and support to clients with a wide range of physical and psychological conditions.

Key Responsibilities

  • To contribute to the assessment, planning, implementation, and review of individual rehabilitation and care programmes for clients.
  • To independently implement these rehabilitation and care programmes as appropriate in the patients' property.
  • To treat clients, often unsupervised, according to agreed care plans and protocols, independently making appropriate adjustments to the rehabilitation programme as required.
  • To be responsible for monitoring clients' progress and amend clients' programmes independently in order to maximise rehabilitation progression and patient safety.
  • To report back to the relevant professional or duty therapist or @risk nurses as required, regarding any changes in the clients' condition following the local escalation procedure.

Requirements

  • Relevant significant experience working in a health, education, or social care setting.
  • Relevant significant experience working with adults and older people with a physical disability for at least one year.
  • Understanding of a wide range of physical and psychological conditions and their impact on a client's ability to undertake every day activities, mobility, and communication.
  • A basic understanding of Occupational Therapy, Speech and Language Therapy, Physiotherapy, and Social Work, and the role of a support worker/Therapy Assistant in supporting the rehabilitation and assessment team.
