Job Title: Senior Rehabilitation Support Worker

We are seeking a highly skilled and experienced Senior Rehabilitation Support Worker to join our team at Guy's and St Thomas' NHS Foundation Trust. As a Senior Rehabilitation Support Worker, you will play a vital role in providing high-quality care and support to our patients, helping them to achieve their rehabilitation goals and regain independence.

About the Role

As a Senior Rehabilitation Support Worker, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing support and guidance to patients and their families.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop individualized care plans
  • Provide high-quality care and support to patients, including assistance with daily living activities, mobility, and communication
  • Work collaboratively with multidisciplinary teams to ensure seamless care and support
  • Monitor patients' progress and make adjustments to care plans as needed
  • Provide education and training to patients and their families on rehabilitation techniques and strategies
Requirements

To be successful in this role, you will need to have:

  • Relevant experience working in a healthcare setting, preferably in a rehabilitation or support worker role
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Basic IT skills, including Microsoft Office and email
  • Ability to lift and move patients as required
