Job Summary

The Extended Primary Care Services team at East London NHS Foundation Trust is seeking a motivated and enthusiastic Rehabilitation Support Worker to join their multidisciplinary team. As a key member of the team, you will work closely with physiotherapists and occupational therapists to provide rehabilitation programs to patients in their own homes.

Main Responsibilities
  • Assist physiotherapists and occupational therapists in providing treatment to patients
  • Carry out rehabilitation programs with patients to maximize their mobility and independence in daily activities
  • Work closely with the multidisciplinary team to provide comprehensive care to patients

Requirements
  • Experience working with older people or people with disabilities
  • Experience of client-centered working within a multidisciplinary team
  • Ability to manage own workload and work flexibly
  • Good interpersonal and written communication skills
