About the RoleThis is an exciting opportunity to work in a dynamic and supportive team, providing rehabilitation and support services to adults with physical disabilities. You will work closely with our multidisciplinary team, including occupational therapists, physiotherapists, and nurses, to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities- Contribute to the assessment, planning, implementation, and review of individual rehabilitation and care programs for clients.
- Independently implement rehabilitation and care programs as appropriate in the patient's property.
- Teach prescribed exercise programs, practice transfers and mobility, assist and promote independence in personal care and domestic tasks, and undertake access visits to feedback environmental information to therapists.
- Treat clients, often unsupervised, according to agreed care plans and protocols, making appropriate adjustments to the rehabilitation program as required.
- Monitor clients' progress and amend clients' programs independently to maximize rehabilitation progression and patient safety.
- Report back to the relevant professional or duty therapist or @risk nurses as required, regarding any changes in the client's condition following the local escalation procedure.
- Complete training to assess for and provide basic equipment to support patient safety.
- Seek timely advice and support as appropriate from senior practitioners or supervisors.
- Work as part of a flexible multidisciplinary team and undertake tasks delegated by the occupational therapists, physiotherapists, nurses, and social workers.
- Communicate effectively with clients and their carers, particularly clients with impairments that may affect their ability to communicate their needs and/or give informed consent to treatment.
- Support and develop RSWs within a competency framework.
- Supervise RSWs with support from senior staff.
- Participate in the prioritization and triage of referrals, based on given criteria and using departmental protocols.
- Participate in clinical governance and service development activities and take a lead role for a delegated service development area.
We are looking for a highly motivated and experienced individual who possesses the following skills and qualifications:
- Understanding of a wide range of physical and psychological conditions and their impact on a client's ability to undertake everyday activities, mobility, and communication.
- A basic understanding of occupational therapy, speech and language therapy, physiotherapy, and social work, and the role of a support worker/therapy assistant in supporting the rehabilitation and assessment team.
- Competent in the use of basic IT, including MS Word, email, internet, and other administrative tasks.
- Fit and able to perform duties requiring a moderate to intense level of physical activity over a sustained period of time, including travel around the borough and clinical tasks.
- Able to demonstrate and use planning, organizational, and time management skills to prioritize and organize client caseloads and competing time and workload demands.
- Ability to communicate and negotiate effectively with clients, carers, and colleagues to achieve the best outcomes for patients.
- Ability to work autonomously and progress/make adjustments to client care/rehabilitation programs.

- Relevant significant experience working in a health, education, or social care setting.
- Relevant significant experience working with adults and older people with a physical disability for at least one year.
- Experience of working in a rehabilitation setting.
- Experience of community/lone working.
- Experience of contributing to service development or training.
