About the Role

The Extended Primary Care Services team at East London NHS Foundation Trust is seeking a skilled Rehabilitation Support Worker to join our multidisciplinary team. As a Rehabilitation Support Worker, you will play a crucial role in providing rehabilitation programs to patients in their own homes, working closely with physiotherapists and occupational therapists.

Main Responsibilities

Key responsibilities of this role include assisting physiotherapists and occupational therapists in providing treatment to patients, carrying out rehabilitation programs as devised by the physiotherapist/occupational therapist, and maximizing patients' mobility and independence in daily activities. You will also be responsible for working closely with the multidisciplinary team and liaising with local GPs to provide community health services for adults living at home.

Requirements

  • Experience working with older people or people with disabilities
  • Ability to manage own workload, work flexibly, and use initiative to problem solve
  • Excellent interpersonal and written communication skills
  • Ability to work as part of a multi-disciplinary team
  • Ability to recognize boundaries of own skill level and seek help as appropriate
  • Basic knowledge of medical conditions, including physical health problems and their impact on daily life
