Rehabilitation Support Worker

We are seeking a skilled Rehabilitation Support Worker to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and rehabilitation services to adults in the community.
About the RoleThe successful candidate will be responsible for implementing rehabilitation and care interventions to clients in their homes, working closely with senior clinicians and other healthcare professionals to develop and implement individualized care plans. You will also be required to communicate effectively with clients, their carers, and family members, particularly those with impairments that may affect their ability to communicate their needs.
Key Responsibilities- Implement rehabilitation and care interventions to clients in their homes, including teaching or supporting prescribed exercise programs, practicing transfers and mobility, and assisting with personal care and domestic tasks.
- Work as part of a flexible, multidisciplinary team, undertaking delegated tasks from the Flow Coordinator and qualified practitioners across all disciplines.
- Communicate effectively with clients, their carers, and family members, particularly those with impairments that may affect their ability to communicate their needs.
- Maintain close working relationships with multidisciplinary team practitioners to ensure effective and timely communication regarding patient care.
- Notify the appropriate Senior Practitioner immediately of any changes in health of clients to ensure their safety and wellbeing at all times.
- Report any changes in progress of clients to ensure that favourable outcomes are met.
We are looking for a candidate with the following skills and experience:
- Evidence of literacy and numeracy to GCSE or equivalent.
- NVQ II or equivalent skills in relevant subjects, such as biology, social care, or a Foundation Degree in Health and Social Care.
- Previous experience of working with adults with disabilities or significant health needs.
- Experience in administrative work, including filing, scanning, and message-taking.
- Experience of working within a multidisciplinary team.
- Fitness to perform duties requiring a moderate to intense level of physical activity over a sustained period of time, including travel around the borough and clinical tasks.
