Hospital Discharge Social Worker

About the Role
Our Hospital Discharge Assessment Team (HDAT) provides essential social care support to residents of North Somerset who require ongoing assistance. We work closely with individuals preparing to leave hospital after an inpatient stay, those transitioning from community-based Discharge to Assess (D2A) services following a recent hospital discharge, and those leaving the Reablement Service after both hospital and D2A discharges.
The HDAT is comprised of experienced professionals including Senior Social Workers, Social Workers, Occupational Therapists, and Adult Social Care Workers. We conduct comprehensive assessments under the Care Act 2014 and Mental Capacity Act 2005 to identify eligible needs and develop personalized care and support plans. Our goal is to ensure individuals receive effective long-term care while advocating for their best interests.
We prioritize a strengths-based approach, empowering individuals to utilize their own abilities and support networks alongside professional care. Our team operates in a fast-paced environment, committed to facilitating timely discharges and minimizing hospital stays.
The HDAT embraces a flexible work model, allowing for a balance between working from home and our offices in Clevedon. Most assessments are conducted in the community, within care homes, or directly in individuals' homes. We also maintain a rota to provide Social Work presence in acute hospitals serving our area. This ensures support for the 'Home First' model within Transfer of Care Hubs, enabling effective discharges and addressing complex needs that may not be met through traditional health D2A routes.
Our team is dedicated to collaborating with system partners to explore appropriate and timely options for individuals returning to a fulfilling life after an acute illness or hospital admission.
