About the Role

We are seeking an experienced Social Worker to join our Hospital Discharge Assessment Team (HDAT) in North Somerset. As a key member of our team, you will play a vital role in providing adult social care support to residents of North Somerset who have ongoing social care needs.

Key Responsibilities
  • Conduct comprehensive assessments to identify eligible needs and develop care and support plans to ensure individuals' needs are met effectively.
  • Work collaboratively with multidisciplinary teams, including healthcare professionals, to ensure timely discharges and minimize days lost.
  • Advocate for individuals and their families, working positively with carers and other professionals to achieve the best outcomes.
  • Operate in a strengths-based way, empowering individuals to draw on their own abilities and support networks for a quality of life.
Team Environment

Our team follows a flexible worker model, allowing for a balance of desk time and community-based work. You will be part of a rota to provide a Social Work presence in acute hospitals, supporting the 'Home First' model and facilitating effective discharges.

Requirements
  • Valid driving licence and own transport, or alternative suitable method of travel.
  • Enhanced Disclosure and Barring Service check.
