About the Role

The Hospital Discharge Assessment Team (HDAT) at North Somerset Council is seeking an experienced Social Worker to join our progressive team. As a result of a new project to provide a Social Work presence in our acute hospitals, we are looking for a skilled professional to support timely discharges and ensure effective care provision.

Key Responsibilities

  • Undertake Care Act assessments, mental capacity, and best interest assessments to identify eligible needs and produce care and support plans.
  • Work proactively with individuals, family members, carers, and other professionals to ensure effective discharges and optimal outcomes.
  • Advocate for individuals and support them in drawing on their own abilities and support networks for a quality of life.
  • Work in a fast-paced environment to support timely discharges with minimal days lost.

Team Dynamics

The HDAT team follows the flexible worker model, allowing for a balance of desk time and community-based work. Staff work collaboratively to support each other and provide a high-quality service to our service users.

Requirements

  • Valid driving licence and own transport, or alternative suitable method of travel.
  • Enhanced Disclosure and Barring Service check.

Working Arrangements

This position is currently based at our offices in Clevedon, with the possibility of relocation to Weston-super-Mare or within North Somerset in the future.
