About the Role:

We are seeking an experienced Advanced Clinical Practitioner to join our team at Dorset Clinical Commissioning Group. As a key member of our multidisciplinary team, you will be responsible for providing high-quality clinical care to patients and carers, while also contributing to the development and implementation of clinical policies and guidelines.

Key Responsibilities:

  • Develop and implement innovative clinical practice, underpinned by the highest level of theoretical and practical knowledge.
  • Undertake comprehensive specialist holistic assessments of patients and carers, planning, implementing, and evaluating care delivery according to changing healthcare needs and varying degrees of clinical risk.
  • Authorise decisions to admit service users and/or initiate discharge, as applicable.
  • Work autonomously using advanced levels of history taking and examination skills within multidisciplinary teams, guidelines, and protocols.
  • Utilise advanced clinical skills in assessment and/or formulate a diagnosis and/or treatment plan.
  • Assess critically and re-evaluate the clinical situation as the patient's condition or needs change, including effective management of risk.
  • Anticipate potential physical and psychological problems caused by the condition or treatment.
  • Advise and communicate with acute hospitals, primary and social care, and community teams to ensure seamless continuity and transfer of care.
  • Act as an advocate for service users, carers, and relatives, empowering and facilitating other professions to meet care needs.
  • Establish, maintain, and effectively manage barriers to advanced, highly skilled, and effective communication with service users, carers, and professionals.
  • Collect, collate, evaluate, and report information, maintaining accurate confidential patient records to a high standard.
  • Develop, implement, and audit protocols, guidelines, and policies for the service.
  • Maintain a professional portfolio and demonstrate up-to-date practice and evidence-based care.
  • Ensure own compliance with mandatory training and revalidation requirements.
  • Identify personal developmental and educational needs to work at an advanced level.
  • Receive clinical supervision to improve knowledge and quality of care.

Person Specification:

Knowledge, Skills, and Training:

  • Demonstrable knowledge of legislation, guidelines, and best practice relevant to the area of clinical practice.
  • Effective leadership skills, including clinical professional leadership and mentorship.
  • Evidence of knowledge and application of national work and standards in relation to advanced clinical practice and the relevant specialty.
  • Demonstrable experience of working in a provider organisation and within a relevant specialty as a Registered Practitioner Band 7 level.
  • Highly developed expertise and knowledge underpinned by theory acquired through CPD in relation to clinical assessment, treatment, and evaluation.
  • Effective leadership and risk management skills, with evidence of relevant leadership in meeting clinical governance objectives.
  • Wide knowledge of health policy and its application in practice, with evidence of developing involvement in influencing local and national standards.
  • Advanced skills for assessing and interpreting complex client conditions, comparing a range of options, and taking appropriate action.
  • Experience of co-ordinating complex activities/programmes of care with other professionals and agencies.
  • Involvement in the development of programmes of care, protocols, and audit.
  • Experience of interdisciplinary working.
  • Innovation and effective use of resources.
  • Active research for the potential to improve quality and customer care, seeking new ideas and methods to improve healthcare.
  • Involvement in policy and leading practice change.
  • Demonstration of publications, presentations, and research at national and international events within the healthcare arena.

Managerial/Supervisory Experience:

  • Proactive involvement in the supervision and development of members of the multi-disciplinary team, with evidence of changes of practice through these.
  • Designing, delivering, and evaluating education and training in practice.
  • Undertaking clinical supervision and systematic peer caseload review of colleagues on an individual or group basis, and supporting aspiring and trainee Advanced Clinical Practitioners through supervisory roles.
