Job Overview

We are seeking a highly skilled and compassionate Care Specialist to join our team at Farleigh Hospice. This is an excellent opportunity for a motivated individual to make a meaningful difference in the lives of patients and their families.

About the Role

As a Care Specialist, you will work within a multi-disciplinary team providing clinical care, navigation, coordination, and support to hospice patients and their families. You will conduct holistic assessments, provide information, advice, psychosocial, and spiritual care to patients with palliative and end-of-life care needs, supporting their families and carers, and liaising with health and social care professionals as necessary.

Main Responsibilities

  • Work collaboratively with the multi-disciplinary team to deliver high-quality patient care
  • Conduct thorough assessments and develop effective care plans
  • Provide emotional support and guidance to patients and their families
  • Liaise with healthcare professionals to ensure seamless care delivery
  • Participate in training and mentoring of junior staff members

Requirements

  • Degree in Nursing or related field
  • Valid nursing registration
  • Experience in palliative care or related disciplines
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
