Care Needs Specialist Position Available
We are seeking an experienced Care Needs Specialist to join our team in Chelmsford, Essex. As a Care Needs Specialist, you will play a key role in assessing, managing, and reviewing the care needs of adults in line with policy, procedures, and legislation.
Key Responsibilities:
- To conduct thorough assessments of care needs, identifying areas for improvement and developing appropriate care plans.
- To establish strong relationships with service users, their families, and carers, promoting independence and maximising potential.
- To prepare and present clear assessments and reports, ensuring that all stakeholders are informed and involved.
Requirements:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Significant frontline experience in Adults Services.
What We Offer:
- A competitive salary of £35,000 - £45,000 per annum (dependent on experience).
