Main Responsibilities- Undertake assessments for people referred into the service for physical health monitoring when commenced on anti-psychotic medication, including depot and Clozapine.
- Manage a caseload of service users with a range of complex needs, developing care plans and delivering evidence-based interventions and treatments.
- Work under the supervision of the clinical lead and have support from the Community Mental Health team multi-disciplinary team.
- Manage unplanned patient work within the team, responding to escalation of risk/crisis situations and stepping up care to a CMHT Band 6 Practitioner as required.
