About the Role

This is an exciting opportunity to join our team as a Community Mental Health Practitioner, working in a busy service that provides a range of services including secondary mental health care. As a key member of our team, you will be responsible for managing a caseload of service users with complex needs, developing care plans, and delivering evidence-based interventions and treatments.

Main Responsibilities
  • Undertake assessments for people referred into the service for physical health monitoring when commenced on anti-psychotic medication, including depot and Clozapine.
  • Manage a caseload of service users with a range of complex needs, developing care plans and delivering evidence-based interventions and treatments.
  • Work under the supervision of the clinical lead and have support from the Community Mental Health team multi-disciplinary team.
  • Manage unplanned patient work within the team, responding to escalation of risk/crisis situations and stepping up care to a CMHT Band 6 Practitioner as required.
