In this role, you will work as part of our multidisciplinary healthcare team to deliver high-quality patient care. You will be responsible for assessing, planning, implementing, and evaluating care plans, while collaborating with other healthcare professionals to ensure seamless delivery of care. Your expertise will contribute to the development of individualized care plans, which will promote the physical, emotional, and social needs of our residents.

Key Responsibilities:

Your key responsibilities will include:

  • Developing and implementing individualized care plans in collaboration with the healthcare team
  • Conducting thorough assessments and evaluations to inform care decisions
  • Administering medication safely and effectively
  • Maintaining accurate and up-to-date records of patient information

Requirements:

To be successful in this role, you should have:

  • Current NMC registration
  • Good understanding of regulatory frameworks, including DoLs/MCA and Royal Pharmaceutical guidelines
  • Experience in producing well-developed care plans and detailed risk assessments
  • Excellent communication and interpersonal skills

Salary:

Our salary range for this position is approximately £35,000 - £50,000 per annum.
