Responsibilities will include:

  • Care and management of patients with long term conditions
  • Collaboration with GPs and other healthcare professionals to develop effective treatment plans
  • Maintenance of accurate patient records and documentation
  • Contribution to quality improvement initiatives and clinical governance activities

We offer a competitive salary of £35,000 - £45,000 per annum, dependent on experience, and a range of benefits including professional development opportunities and a supportive working environment.
