A highly motivated and experienced nurse is required to join our team in a fast-paced primary care environment.
About the OpportunityThis is an exciting opportunity for a skilled nurse to utilize their expertise in managing long term conditions and contribute to the delivery of high-quality patient care.
The ideal candidate will have experience in working within a GP practice, with a strong understanding of long term conditions management and excellent organizational skills. They will be able to work independently and as part of a team, prioritizing tasks effectively and maintaining strict confidentiality.
Responsibilities will include:
- Care and management of patients with long term conditions
- Collaboration with GPs and other healthcare professionals to develop effective treatment plans
- Maintenance of accurate patient records and documentation
- Contribution to quality improvement initiatives and clinical governance activities
We offer a competitive salary of £40,000 - £50,000 per annum, dependent on experience, and a range of benefits including professional development opportunities and a supportive working environment.
