About the Role

We are seeking a highly skilled and experienced Dementia Nurse to join our team at Cornwall Partnership NHS Foundation Trust. As a key member of our Memory Assessment Service, you will play a vital role in providing specialist nursing care and support to individuals with dementia and their families.

The successful candidate will have a proven track record of working with people with dementia, managing complex cases, and providing expert advice to primary care teams. You will work closely with multidisciplinary teams, including consultants, GPs, and other healthcare professionals, to deliver high-quality patient care.

Key Responsibilities:

  • Provide specialist nursing care and support to individuals with dementia and their families
