Complex Care Specialist

We are seeking an experienced Complex Care Specialist to join our team. The successful candidate will be responsible for providing complex care and support to individuals in need, working flexible shifts, including nights and weekends, and traveling between Newquay, Truro, and surrounding areas as required.
Key Responsibilities:
- To assess and implement care plans that meet individual needs
- To develop and maintain relationships with service users, their families, and external partners
- To ensure compliance with regulatory requirements and quality standards
Requirements:
- A minimum of experience in complex care, preferably with a focus on case management
- A valid driver's license (ideally)
- Ability to commute to Newquay and surrounding areas
Salary and Benefits:
- £15.30 per hour, paid weekly
