Job DescriptionIn this role, you will work closely with our service users, their families, and other healthcare professionals to develop and implement personalized support plans that promote independence, autonomy, and personal growth. Your key responsibilities will include:
- Conducting assessments and developing support plans that meet the unique needs of each individual.
- Providing enabling care that promotes independence and well-being.
- Planning and delivering engaging activities that foster social interaction, skill-building, and personal growth.
- Maintaining accurate and up-to-date records of service user information and support plans.
