Spencer Clarke Group is seeking a dedicated professional to fill the role of Care Transition Specialist. This position is based in East London and involves providing essential support and guidance to young people transitioning from care to independence.

About the role:

The successful candidate will be responsible for managing a caseload of young people, developing and reviewing Pathway Plans to support their journey to independence. This will involve conducting timely Pathway Assessments in collaboration with young people, carers, and social workers.

The ideal candidate will have experience working directly with children in care and care leavers, both individually and in group settings. They will also have experience of undertaking high-quality and analytical Pathway Assessments, Human Rights Assessments, and Personal Educational Plans for looked-after children and young people.

About you:

The successful candidate will have the following skills and experience:

  • Experience working directly with children in care and care leavers, both individually and in group settings.
  • Experience of undertaking high-quality and analytical Pathway Assessments, Human Rights Assessments, and Personal Educational Plans for looked-after children and young people.
  • Experience working with young people who have faced social exclusion within a multicultural, inner-city environment.
  • Experience in supporting young people through their transition to independence.

What's on offer:

Salary: £19+ph

Contract type: 3-6 months minimum, with a high likelihood of extension

Hours: Monday to Friday, 36 hours per week
