About the Role

We are seeking an experienced and qualified Social Worker to join our Continuing Healthcare team in Hampshire County Council.

Job Description

As a registered Social Worker, you will work collaboratively with multidisciplinary teams to support individuals and their families in transitioning to suitable care provision following assessment.

You will be responsible for undertaking assessments in line with the Care Act 2014, supporting individuals with their onward care journey, and engaging with professionals to complete NHS CHC assessments.

Our ideal candidate will have a strong understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, as well as experience working within Adult Social Care in the UK.

Required Skills and Qualifications

  • Social Work qualification and registration with Social Work England (SWE)
  • UK based experience within Adult Social Care
  • Experience and/or knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care
  • A sound understanding of how to apply a strengths-based approach
  • Understanding of social work issues, legal frameworks, and social services' statutory duties

Benefits

This is a hybrid role with 20% of your week spent in office. You will be required to travel across Hampshire and attend regular team meetings in Basingstoke.

Our estimated salary for this position is £35,000 - £45,000 per annum, depending on experience.
