  • Delivering Comprehensive Assessments

As a Clinical Triage Specialist with Norfolk Community Health and Care NHS Trust, you will undertake comprehensive assessments of patients referred to the pulmonary rehabilitation service to determine their baseline and discharge clinical outcome measures. This will involve working in a variety of settings and collaborating with the wider multi-disciplinary community pulmonary rehabilitation team.

  • Triage of Referrals

You will be required to assist with the triage of referrals to the pulmonary rehabilitation service effectively and efficiently, ensuring minimal delay in meeting the patient's needs and meeting the agreed key performance indicators. This will involve assessing referrals and determining the most appropriate course of action.

  • Risk Assessments and Infection Control

As a member of the pulmonary rehabilitation team, you will be required to undertake risk assessments and act upon them appropriately. You will also be responsible for adhering to infection control policies and utilising universal precautions when required.

  • Record-Keeping and Data Management

You will be responsible for recording activity data and clinical information on SystmOne and completing it on a daily basis. This will involve ensuring accurate and timely recording of patient data and information.

  • Training and Development

As a Clinical Triage Specialist, you will be required to attend in-house training as required to develop your current job role. You will also be responsible for maintaining your own continuous professional development and staying up-to-date with current guidelines and standards for pulmonary rehabilitation and respiratory condition management.

  • Mentorship and Support

You will be expected to provide mentorship for students undertaking pre/post-registration courses and provide advice/training to less experienced staff. You will also be responsible for supervising and supporting other staff and students where appropriate.

  • Driving Requirements

As a member of the pulmonary rehabilitation team, you will be required to hold a full and valid driving licence and have access to a vehicle. You will be responsible for making your own travel arrangements to pulmonary rehabilitation programme venues, bases, and meetings as required.

  • Qualifications and Experience

Applicants should hold a degree in Physiotherapy or Sports therapy and evidence of continuing professional development. A COPD diploma/Pulmonary rehabilitation diploma or similar/equivalent is desirable, as is experience of working in a community setting and awareness of community services.

  • Communication and Interpersonal Skills

Excellent communication and interpersonal skills are essential for this role, as you will be required to work effectively with patients, colleagues, and other healthcare professionals. You should be able to communicate effectively in written and verbal English Language and be willing to undertake appropriate training.

  • Personal Attributes and Behaviours

You should be able to identify with the Trust's commitment to safeguarding and promoting the welfare of children and young people/vulnerable adults. A flexible and positive approach to meeting the needs of the service is also essential.
