About the Role

We are seeking a highly motivated and experienced Social Worker to join our Transitions Team. The successful candidate will be responsible for supporting young people as they transition from children's services to adulthood, working with them and their families to plan for their future and ensure a smooth transition.

The ideal candidate will have a strong understanding of safeguarding and be able to work creatively with young people and their families to develop effective transition plans. They will also have experience of working with vulnerable adults and be able to provide support and guidance to help them navigate the complex route into adult services.
